Licorice root contains glycyrrhizin — a compound 30-50x sweeter than sugar that inhibits 11β-HSD2 (the enzyme that inactivates cortisol in the kidneys). This causes cortisol to accumulate and activate mineralocorticoid receptors → sodium retention → hypertension → hypokalemia. At normal consumption (<100mg glycyrrhizin/day, equivalent to ~50g licorice) effects are minimal. At higher doses: pseudoaldosteronism (mimics excess aldosterone).
