Horse chestnut seed extract (HCSE) containing aescin is one of the most evidence-backed herbal treatments for chronic venous insufficiency (CVI) — heavy legs, swelling, varicose veins. A Cochrane review of 17 RCTs found it reduced leg pain, edema, and leg volume comparably to compression stockings. Our research shows aescin has a unique mechanism: it specifically seals "leaky" capillaries by strengthening the vascular endothelium, reducing the fluid leak that causes edema. Venostasin® is the gold standard extract used in most European trials.
Aescin (a mixture of triterpene saponins) works through: (1) capillary permeability reduction — aescin inhibits the enzyme hyaluronidase, which degrades the "mortar" (glycosaminoglycans) between endothelial cells, strengthening capillary walls; (2) venous tone increase — aescin stimulates PGF2α release, causing venoconstriction (tightening of venous walls); (3) anti-inflammatory — inhibits phospholipase A2, reducing prostaglandin and leukotriene production; (4) lymphatic drainage improvement. The net effect: less fluid leaks out of capillaries (reducing edema) and veins are tighter (reducing blood pooling and varicose appearance).
